Terms of Service & Privacy Policy

Oakman Physical Therapy provides in-clinic, one-on-one physical therapy services at 5237 Oakman Blvd., Suite 100, Dearborn, MI 48126. All care is delivered by or under the supervision of licensed physical therapy professionals.

By scheduling an appointment or receiving treatment, you agree to these terms. If you do not agree, please do not use our services.

We ask for at least 24 hours' notice if you need to cancel or reschedule. This allows us to offer your slot to another patient.

• Late cancellations (under 24 hours): No fee
• No-shows: No fee
• First missed appointment: Automatically waived as a one-time courtesy

For established patients: three (3) consecutive missed appointments without notice will result in discharge from your active treatment plan. No financial penalty applies. You are welcome to contact us to restart care with a new evaluation.

We accept most major insurance plans and also see patients on a self-pay basis. We work with auto accident and No-Fault insurance cases.

You are responsible for knowing your plan's co-pays, deductibles, and coverage limits. Any balance not covered by your insurer remains your responsibility. Payment is due at the time of service unless other arrangements have been made.

Our services are physical therapy — not medical diagnosis or the practice of medicine. Results vary by individual and are not guaranteed. The content on our website is for educational purposes only and does not replace advice from your physician or qualified healthcare provider.

In an emergency, call 911 or go to your nearest emergency room.

To make your treatment as effective as possible, we ask that you:

• Arrive on time and notify us if you need to cancel
• Share your full medical history and any changes in your condition honestly
• Follow your home exercise program as instructed
• Treat our staff and other patients with respect

We reserve the right to refuse or discontinue services to anyone who is disrespectful, disruptive, or presents a safety concern.

We may update these terms from time to time. Changes will be posted on this page with an updated effective date. Continued use of our services means you accept the updated terms.

When you contact us, book an appointment, or fill out a form on our website, we may collect:

• Your name, phone number, and email address
• Basic information about your condition or reason for visit
• Insurance information provided at intake
• Health information shared during your care (protected under HIPAA)

We use your information to:

• Schedule and confirm your appointments
• Provide and coordinate your physical therapy care
• Submit insurance claims on your behalf
• Follow up with you about your treatment or upcoming visits
• Respond to inquiries submitted through our website or forms

We do not sell your personal information. We do not share it with third parties for marketing purposes.

Your protected health information (PHI) is handled in accordance with the Health Insurance Portability and Accountability Act (HIPAA). We use and disclose your PHI only for treatment, payment, and healthcare operations as permitted by law.

A full Notice of Privacy Practices is provided to all patients at intake. You may request a copy at any time by contacting our office.

Our website may use cookies and tracking tools (including the Meta Pixel) to understand how visitors interact with our site and to improve the effectiveness of our advertising. No personally identifiable health information is collected through these tools.
