Terms of Service

1. Our Services

Oakman Physical Therapy provides in-clinic, one-on-one physical therapy services at 5237 Oakman Blvd., Suite 100, Dearborn, MI 48126. All care is delivered by or under the supervision of licensed physical therapy professionals.

By scheduling an appointment or receiving treatment, you agree to these terms. If you do not agree, please do not use our services.

2. Appointments & Cancellations

We ask for at least 24 hours' notice if you need to cancel or reschedule. This allows us to offer your slot to another patient.

  • Late cancellations (under 24 hours): No fee
  • No-shows: No fee
  • First missed appointment: Automatically waived as a one-time courtesy

For established patients: three (3) consecutive missed appointments without notice will result in discharge from your active treatment plan. No financial penalty applies. You are welcome to contact us to restart care with a new evaluation.

3. Payment & Insurance

We accept most major insurance plans and also see patients on a self-pay basis. We work with auto accident and No-Fault insurance cases.

You are responsible for knowing your plan's co-pays, deductibles, and coverage limits. Any balance not covered by your insurer remains your responsibility. Payment is due at the time of service unless other arrangements have been made.

4. Medical Disclaimer

Our services are physical therapy — not medical diagnosis or the practice of medicine. Results vary by individual and are not guaranteed. The content on our website is for educational purposes only and does not replace advice from your physician or qualified healthcare provider.

5. Patient Responsibilities

To make your treatment as effective as possible, we ask that you:

  • Arrive on time and notify us if you need to cancel
  • Share your full medical history and any changes in your condition honestly
  • Follow your home exercise program as instructed
  • Treat our staff and other patients with respect

We reserve the right to refuse or discontinue services to anyone who is disrespectful, disruptive, or presents a safety concern.

6. Changes to These Terms

We may update these terms from time to time. Changes will be posted on this page with an updated effective date. Continued use of our services means you accept the updated terms.

4. Website & Cookies
